Carrot Fertility

What they typically require
- DONA, CAPPA, or equivalent doula certification
- Current CPR certification
- $1M professional liability insurance
- W-9 + invoicing info under your legal name (LLC not required)
Timeline: Typically 2–4 weeks from application to approved-provider status.
Maven Wallet
mavenclinic.comParental-health benefit used by many large employers across tech, finance, and retail.
What they typically require
- Doula certification
- Current CPR certification
- $1M professional liability insurance
- Background check (Swaddl runs Checkr for you)
Timeline: Typically 2–6 weeks; varies by employer config.
Progyny
progyny.comFertility benefit common at finance, consulting, and large enterprise employers.
What they typically require
- Doula certification
- Current CPR certification
- $1M professional liability insurance
- Provider intake call with their network team

What Swaddl handles, either way
Once you're an approved provider with a program, Swaddl handles invoice formatting, claim submission, and reimbursement tracking for that family — so the most common rejection reasons (wrong invoice format, missing provider info) don't cost you a check.
